Loginless Healthcare Provider Search

Overview

This was part of the goal to make a really cheap search for clients who did not want to fund the cost of a full portal system. I designed it to be nearly black-and-white and with a generic font that could apply to any client. The product was not meant to be customized to clients beyond adding a logo.
